How long to wait after a vaccine before getting a titer.

A titer drawn too soon after vaccination doesn't reliably measure immunity — the antibodies haven't peaked yet. Draw too late for a hepatitis B post-series titer and you might miss a real primary vaccine response. The right window depends on which vaccine you got. This guide gives you the intervals, source-cited, so you can time the blood draw correctly the first time.

Tetanus, diphtheria, pertussis, HPV, meningococcal, flu, and shingles vaccines don't have routine post-vaccination titer testing — documentation of the vaccine itself is the standard. Titers are used routinely only for the four vaccines above.

Why timing matters — the underlying biology

Vaccination triggers a two-part immune response. First, the vaccine antigen replicates or is processed (1–2 weeks after vaccination1 for live vaccines), then your immune system generates antibodies. IgG — the antibody most titer tests measure — builds gradually as the primary response matures. Draw blood before the antibodies have accumulated enough to cross the assay's detection threshold and you get a false-negative result, which triggers a preventable revaccination.

ACIP's General Best Practices puts a fine point on it: "Approximately 90%–95% of recipients of a single dose of certain live vaccines administered by injection at the recommended age (i.e., measles, rubella, and yellow fever vaccines) develop protective antibodies, generally within 14 days of the dose"1. That's the minimum floor. In practice, waiting 4 weeks after the last dose — and ideally 3 months — gives the antibody titer time to stabilize.

Hepatitis B — the tightest window

The Hepatitis B post-vaccination titer is the strictest interval on the list because it's actively used to confirm vaccine response for healthcare workers, dialysis patients, HIV-positive persons, and infants of HBsAg-positive mothers. The CDC Pink Book2 is unambiguous:

"Testing should be performed 1 to 2 months after administration of the final dose of the vaccine series using a method that allows determination of a protective concentration of anti-HBs (greater than or equal to 10 mIU/mL)."

Why not later? Anti-HBs antibodies wane over time — a titer drawn a year or two after the series can read as "negative" even in someone who responded perfectly to the vaccine. This is why the Pink Book explicitly notes that "a negative anti-HBs serologic response in HCP who received HepB vaccine in the distant past will not distinguish between failure to respond to the initial vaccination series (lack of protection) and response to the initial vaccination series with subsequent waning of antibody (protected)"2. If you're a healthcare worker or nursing student and you never got the confirmatory titer after your childhood series, you may have to repeat the whole series just to prove the response — which is why we tell people to get the 1–2 month titer done the first time.

Special case: if you received Hepatitis B immune globulin (HBIG) alongside your vaccine — usually after a needlestick or as a newborn of an HBsAg-positive mother — wait 4 to 6 months after the HBIG dose before testing2. The passive antibodies from HBIG can be picked up by the assay and give a false-positive result if you test too soon.

MMR and varicella — the "at least 4 weeks" rule

Live-virus vaccines (MMR and varicella) require the same minimum 4-week wait, since the vaccine virus itself needs 1–2 weeks to replicate and then more time for the antibody response to develop. In practice most clinicians wait longer:

  • 4 weeks minimum. Sufficient for most commercial assays to detect vaccine-induced immunity if you had the response.
  • 3+ months preferred. The CDC's postpartum guidance specifically instructs providers to test "≥3 months later"1 for women vaccinated after delivery who need documented immunity — the longer window catches slower responders.
  • A key caveat for varicella: the MMWR healthcare-personnel guidance flags that "commercial assays can be used to assess disease-induced immunity, but they often lack sensitivity to detect vaccine-induced immunity (i.e., they might yield false-negative results)"3. Translation: a negative varicella titer after documented vaccination sometimes just means the commercial assay isn't picking up real immunity, not that the vaccine failed. Your clinician may re-immunize based on the titer alone — that's protocol, not necessarily a failed vaccine.

Draw too soon: what happens

The most common expensive mistake in vaccine-and-titer sequencing looks like this:

  1. Nursing student gets Hep B dose 3 on Monday.
  2. Occupational health orders a titer for onboarding paperwork, drawn Wednesday (2 days later).
  3. Anti-HBs comes back <10 mIU/mL. The titer was drawn during the window where the primary response hasn't fully matured.
  4. The employee is told they're a non-responder and needs a repeat 3-dose series and a second titer — a process that takes 6+ months.
  5. The initial series probably worked fine; the titer was just drawn too early.

This is preventable. Wait the full 1–2 months after the final dose for Hep B, wait at least 4 weeks after any live vaccine, and confirm the timing with your ordering clinician before the blood draw.

Titers that don't need a waiting period

If the goal is to document immunity from a vaccine or infection you had in the distant past — say, MMR shots you got as a toddler and you're now applying to nursing school at 22 — you don't need to wait for anything. The antibodies (if present) have long since stabilized. Just get the draw.

The waiting periods above only apply when you've had a recent vaccine dose and are testing specifically to confirm the response to that dose.

Where to get titers drawn

Titers are ordinary lab draws — no special preparation needed, no fasting, one tube of blood. Options in rough order of speed:

  • Your primary care office. Slowest but usually covered by insurance. Ask for a quantitative measles/mumps/rubella IgG, varicella IgG, anti-HBs, and hepatitis A IgG panel — that's the standard "immunity panel" most schools and employers accept.
  • Occupational health at your new employer. If you're onboarding at a hospital or clinical program, they'll typically draw all required titers as part of the pre-employment health assessment.
  • Urgent care or independent lab-draw clinic. Often the fastest same-week option if you can pay out of pocket or have PPO insurance.
  • Direct-to-consumer testing services. Order the specific titer online, receive a lab order, walk into a nearby draw station, get results in 3–5 business days. Whichever route you take, confirm the report your school or employer expects (numeric quantitative value vs. positive/negative) before ordering.

A quick summary

  • Hepatitis B: 1–2 months after final dose. If you had HBIG, add 4–6 months.
  • MMR, varicella, hepatitis A: at least 4 weeks after the last dose; 3+ months is more reliable.
  • Old vaccines from childhood: no waiting period — draw whenever.
  • Live vaccines and TB tests: different problem, same-day or 4-week wait (see the healthcare worker guide).
  • Ask your clinician if in doubt. A wrong-timing titer that comes back negative is the single most common reason an otherwise-immune person ends up getting revaccinated.
